Pacific Western University Form

Greetings all,

Pacific Western University, when it existed, used to enclose a form, which was akin to CV template, in their application mailings. I believe they used the form as basis for experiential credit. I used to have a copy of it but now I can't locate it.

Does anyone out there have the form still, and might be willing to share a copy of it? I remember that Dr. Bear in one of his earliest books--when PWU was authorized and legal--had spoken well of the form.

I remember it as being a sensible way for a person to categorize and identify his or her formal and informal learning experiences.

Unfortunately my copy is in the New Jersey landfill, along with most of 'my' other older files, which went to Pearson Inc. when I sold them my copyrights in 1998 and shipped everything off to the World Trade Center.
